Participant Criteria
To be eligible for the Eureka Adult Foster Care program, participants must meet the following criteria:
- Age: Participants must be at least 16 years old.
- Assistance with ADLs: Participants require assistance to complete Activities of Daily Living.
- Supervision: Participants need supervision for at least 21 hours daily.
- Medical Condition or Cognitive Impairment: Participants must have a medical condition or cognitive impairment that necessitates hands-on assistance or active supervision and coaching in at least one of the following activities: Bathing, Dressing, Toileting, Transferring, Mobility, or Eating.
